A child’s relative, NREFM, or Family friend can use the JV-285 form to give information to the court. This can provide detailed information about the child’s specific needs or circumstances to the judge. The form can include information about your relationship with the child, the child’s education or medical needs, and any other information that might be helpful to the court.
A relative or NREFM may also use the JV-285 to let the court know they are interested in visitation or placement of the child or youth in foster care.
- A “non-relative extended family member – NREFM” is defined as an adult caregiver who has an established familial relationship with a relative of the child or a familial or mentoring relationship with the child. The county welfare department shall verify the existence of a relationship through interviews with the parent and child or with one or more third parties. The parties may include relatives of the child, teachers, medical professionals, clergy, neighbors, and family friends. Welfare & Institutions Code §§ 362.7, 361.3
When completing the JV-285, be sure to provide be factual specific information, remain child-centered, positive, and kind. You may also attach a letter and can include:
- A photo of the child, so the judge can put a beautiful, smiling face to a name and file number.
- Briefly summarize all the positive developments in the child’s life and the progress made including school and social activity successes and accomplishments.
- Factually share any concerns or individual needs the child may have that are or are not being met.
- If possible, attach letters from doctors, teachers, or other professionals to support your concerns or requests for services.
- If you add a letter or attachments, be sure to check the box on item 12, indicate the total number of pages attached, and include the date and the case number. Label each attachment page “Attachment to JV-285, Date, Case Number.”
When and where do I submit a JV-285?
- Form JV-285 may be submitted at any time during the court process.
- Filing Your JV-285 in Person at the Courthouse – File 8 copies your of your completed JV-285 form with attachments at the Clerk’s office on the 1st floor of the Yolo County court house at least 5 days before the hearing. Be sure to keep one copy stamped “filed” by the Clerk, for your records.
- Filing Your JV-285 by Mail – Mail 8 copies of your completed JV-285 form with attachments to the Yolo County’s juvenile court filing clerk’s office at least 7 days prior to the hearing date. Include a cover letter indicating the mailing is for “filing and service” and that you request a “file-endorsed” copy returned in the self-addressed stamped envelope you have provided.
