Yolo County’s Children’s Services Coordinator – call the main reception line and ask for Children’s Services Coordinator #
The Coordinator is the first line of support for Foster Caregivers and can help with:
- Social worker and supervisor contact information
- Contact information for the minor’s attorney
- Emergency/temporary (30 day) Medi-Cal cards
- Referrals and assessments for special services (i.e referrals for child victim counseling, children 0-5 developmental screenings, childcare…)
- Can help schedule an medical exam within 30 days of placement at the CommuniCare Hansen Family Health Center in Woodland.
- Will help get healthcare information from biological parents as available.
Foster Care Reimbursement #

Medi-Cal Cards #
New Cases – Temporary (30 day) Medi-Cal cards
- Please contact the Yolo Children’s Service Coordinator (see above) and a temporary card will be emailed to you. Please include the youth’s legal dependency name, date of birth, and current address.
All Ongoing Cases – Temporary (30 day) or Physical Medi-Cal cards
Please email the eligibility team at yolo.care@yolocounty.org and include the youth’s legal dependency name, date of birth, and current address.
- Temporary Medi-Cal card will be emailed to you typically in 1-3 days.
- Physical Medi-Cal card is issued by the state and mailed to the youth’s placement address typically within 2+ months of request.
- Foster youth in extended foster care can request Medi-Cal cards from their social worker or the youth can directly email the eligibility team.
Yolo County Public Health Nurses (PHN) #
The PHN’s can help support foster youth’s complex medical care needs. For assistance, please email hhsa.cyf-fostercare-nurses@yolocounty.org
ILP (Independent Living Program) #
ILP supports current and former foster youth (14 ½ – 21) to have a positive community and gain life skills necessary in order to become more self-sufficient and successful as adults. Yolo County Child Abuse Reporting Hotline – 530-669-2345 #
RFA caregivers are mandated reporters of suspected child abuse and neglect. Please complete the Suspected Child Abuse Report form before you call the hotline to expedite the process. After you have completed your verbal report, please note the Social Worker’s name who took your call on your completed Suspected Child Abuse Report form and email the completed document to ChildAbuseReport@yolocounty.org or FAX to 530-661-6012.
- Reports should be as detailed as possible, but please do not investigate.
- Provide information on the child and family’s support network.
- Include, if known, if the parent is taking any steps at ensuring the child’s safety or if the parent and child have any strengths that CWS can rely upon to help in safety planning with the family.
