All foster youth qualify for Medi-Cal
Medical Care
- For all foster youth, the foster caregiver must schedule a Child Health & Disability Prevention Exam (CHDP) within 30 days of placement.
- The medical provider or foster parent must complete the Foster Care Report of Medical Exam form and return the form by email to email to hhsa.cyf-fostercare-nurses@yolocounty.org or FAX at 530-661-3630.
- Major healthcare providers including Sutter Health, Kaiser Permanente, and Dignity Health accept Medi-Cal. CommuniCare Health Centers throughout Yolo county also provide pediatric health care for Medi-Cal youth, see details
- If you need assistance scheduling an exam with a local pediatrician, the Yolo County’s Children’s Services Coordinator can schedule an exam at the CommuniCare health Center in Woodland.

Dental
- For foster youth older than 1 year, the foster caregiver must schedule a Dental Exam with within 30 days of placement.
- The dental provider or foster parent must complete the Foster care Dental Contact Form and return the form by email to hhsa.cyf-fostercare-nurses@yolocounty.org or FAX at 530-661-3630.
- MediCal dental coverage includes 2 dental exams per year.

Vision – Schedule as needed. For example, if you, the youth’s doctor, or a teacher suspects a need for glasses.
Medi-Cal vision benefits include a routine eye exam and eyeglasses once every 24 months.
Mental Health Counseling – Typically, the youth’s social worker will make a referral for counseling services with county mental health therapists. The child’s pediatrician may also make a referral for counseling within the child’s medical network (for example, within Dignity, Sutter or Kaiser healthcare system).
